Questionnaire instructions:
The following questionnaire is free and non-binding you to anything.
- Questionnaire needs to be downloaded (Microsoft Word document), save it on your computer, fill out and email it to us as an attachment.
- Please fill out the questionnaire with all requested information, as they affect our knowledge of you as a student and athlete and subsequently your chances of getting an athletic scholarship.
- Please fill out the questionnaire in English language.
- Provide as many details as possible. If you are unsure about an answer, feel free to explain.
- Unsuficiently filled questionnaires will be accepted, but will delay your application, a thing we definitely would like to avoid.
- Provided information will not be used for alternative purposes.
